A 53 year old male presents with palpitations for one hour. He has no chest pain and he is awake and able to talk to you. His BP is 110/ 60. He has no past medical history and has a family history significant only for a brother in his 60’s who had a stroke.
What does the EKG show?
EKG diagnosis: Ventricular tachycardia
Clinical diagnosis: Stable ventricular tachycardia
What intervention did you do to go from the 1st EKG to the 2nd EKG?
-Amiodarone or lidocaine
-if unstable vital signs, immediate cardioversion
-may try adenosine initially if you suspect SVT with aberrancy and vitals are stable
-remember, wide complex tachycardia is Ventricular tachycardia until proven otherwise
