This patient is a 23 year old male who presents to the ED with the chief complaint of knee pain with ambulation. The following X ray is shown.

 Questions:

1. Describe what you see?

    • Knee xray – shows a lucency in the convexsurface of the medial condyle of the knee.
    • Diagnosis: Osteochondritis dissecans (OCD)

  • 2. Physical exam would show:
      • Patients usually report pain at the extremes of motion range.
      • Periarticular edema is often present with slight warmth to the touch.
      • When a lower extremity is involved, patients often limp.
      • Symptoms usually improve with protected immobilization of the joint.
      • Decreased range of motion (ROM), particularly with internal rotation and abduction

    3. How would you manage this patient?

      • Treatment consists of discontinuation of the injurious activity,
      • Protected immobilization of the joint
      • Administration of nonsteroidal anti-inflammatory medications.
      • Surgery may be required to remove the intra-articular loose body and/or correct the resulting degenerative changes.
