This patient is a 23 year old male who presents to the ED with the chief complaint of knee pain with ambulation. The following X ray is shown.
Questions:
1. Describe what you see?
2. Physical exam would show:
- Patients usually report pain at the extremes of motion range.
- Periarticular edema is often present with slight warmth to the touch.
- When a lower extremity is involved, patients often limp.
- Symptoms usually improve with protected immobilization of the joint.
- Decreased range of motion (ROM), particularly with internal rotation and abduction
3. How would you manage this patient?
- Treatment consists of discontinuation of the injurious activity,
- Protected immobilization of the joint
- Administration of nonsteroidal anti-inflammatory medications.
- Surgery may be required to remove the intra-articular loose body and/or correct the resulting degenerative changes.
