A 59 year old male presents to the ED after an episode of passing out. He states that he had shortness of breath, nausea and chest pain for the last 8 hours. He has a PMHx of DM, HTN, and is a smoker. What does the EKG show?
Rate: 50s
Rhythm: sinus but irregular
Axis: normal
PR: progressively prolonged
QRS: narrow complex, occasionally not conducted after a p wave
ST: t-wave flat in aVF and V2, inverted in III, V1 and V3
EKG diagnosis: second degree AV block, type I
Clinical diagnosis: Chest pain r/o acute MI ( AV block often associated with inferior MI)
