             This patient is a 17 year old male presents to the ED with the occciptal headache for several hours. The patient states his headache started after a football game. His father stated his son was playing in the state championship game and drilled his helmet into the opposing player’s chest. The following ray is shown.

1. Describe what you see?

• Lateral displacement of the body of C1 on C2 – Jefferson Burst Fracture

2. How does this injury typically occur?

• Typically caused by an axial-loading force on the occiput of the head

3. What would the physical exam show?

• Patients usually complain of upper neck pain and or headache following trauma
• Neurological examination is usually normal

4. What associated can be seen with this injury?

• Approx 1/3 of Jefferson fractures are associated with a fracture of C2.

 Trivia: Jackson Browne and actress Darryl Hannah lived together for several years in the 80's. One incident caused a lot of media attention when Jackson called the police to report a domestic situation. What singer/songwriter wrote a song about this that accused Jackson of beating up Darryl?

Answer: Joni Mitchell's song "Not to Blame" from the album "Turbulent Indigo" accused Jackson of beating up Darryl. Jackson's response regarding the song was that Joni had a lot of problems and was ill. In my opinion no one really knows what happened the night that Jackson called the police but Jackson and Darryl. No charges were ever filed on either party involved.
