This patient is a 64 year old male who was found by her family sleeping on the floor. When they aroused her she was lethargic and confused. The following CT scan is shown.
Questions:
1. Describe what you see? Blood around the circle of Willis consistent with a subarachnoid hemorrhage
2. Why is a CT scan the preferred diagnostic method for this disease?
The CT scan is the preferable to MRI because it is more sensitive in detecting hemorrhage in the first 24 hours.
3. What should be done if the CT scan is negative?
If the CT scan is negative, in light of the history, a lumbar puncture should be performed to look for xanthochroid and or an infectious cause of the patients mental status change.
4. What is the Hunt - Hess grading system for your diagnosis?
Grade 0 Unruptured aneurysm Grade 1 Asymptomatic or mild headache Grade 2 Moderate-severe headache, nuchal rigidity, ± cranial nerve deficits Grade 3 Confusion, lethargy, or mild focal symptoms Grade 4 Stupor, ± hemiparesis Grade 5 Comatose, ± extensor posturing
