 This patient is an 11 year old male who presents to the ED with the complaint of elbow pain after falling from a skate board. The following ray is shown.

Questions:

1. Describe what you see?

      A subtle fracture of the radial head with joint effusion posteriorly.

                               Diagnosis: Subtle fracture of the radial head

2. What is the mechanism of injury?

         Usually from a fall on the outstretched arm with the force of impact transmitted up the hand through the wrist and forearm to the radial head, which is forced into the capitellum.

3. What are the complications of this injury?

        Early complications of radial head or neck fractures or dislocations include compartment syndrome, neurovascular injury, and infection. Late complications include nonunion, hardware failure, malunion, infection, synostosis, and persistent pain.
