This patient is a 24 year old male who fell off a ladder and landed on his back and is complaining of shoulder pain. The following x ray is shown.
Questions:
1. Describe what you see?
A fracture of the scapula that extends into the glenoid. The glenoidhumeral joint is intact.
2. What are the typical clinical presentations of patients who present with this type of injury?
Localized tenderness over the scapula with the arm held in adduction.
A flattened appearance of the shoulder.
3. What is the usual mechanism of injury?
Usually from a direct blow or trauma to the shoulder sometimes with injury to the acromion or coracoid or a fall on an outstretched arm.
4. What concomitant injuries are possible with this injury?
There is a greater than 80 percent association of injuries to the ipsilateral lung, thoracic cage, and shoulder girdle.
