November 2009 EKG Case
A 70 year old man presents unconscious with a blood pressure of 100/60. He has a history of diabetes and hypertension.
What does the EKG show and what is your diagnosis?
Rate: 155
Rhythm: wide complex tachycardia
Axis: left
PR: no p waves seen
QRS: bizarre wide complex, almost sine wave
ST: no ST elevation
EKG Diagnosis: Ventricular tachycardia
Clinical Diagnosis: Acute MI with Ventricular tachycardia
- Also, consider hyperkalemia with bizarre almost sinusoidal wide complexes
- This patient had serum potassium of 8.1 meq/l
