A 58 year old male presents with lightheadedness when he stands. He is pale and uneasy as the triage nurse escorts him to the stretcher. He denies any chest pain.
What does the EKG show?
Rate: ~ 30
Rhythm: AV dissociation
Axis: Normal axis
QRS: .08
PR: variable, dissociated
ST: no elevation
EKG diagnosis: 3rd degree heart block
Treatment: Pacemaker is definitive. Can try atropine but it is unlikely to work. Lidocaine is NOT indicated.
