October 2008 EKG case:
A 47 year old male presents with crushing chest pain for one hour. He states that he has had acid reflux and thinks he needs an antacid. He has a past medical history of GERD, HTN, DM, dyslipidemia, and CAD. He has a negative exercise stress test 6 months ago. His BP is 90/60.
Rate: 50
Rhythm: sinus bradycardia with 1st degree AV block
Axis: normal
PR: 0.24
QRS: 0.08
ST: ST elevation in II, III, AVF, V4, V6, ST depression in I, AVL, V2, V3
EKG Diagnosis: ST elevation MI
Clinical Diagnosis: Inferior lateral wall MI
Treatment: -oxygen, aspirin, right sided EKG, IV fluids, thrombolytics vs transfer to cath lab
-Do not give Beta blockers or nitrates with possible RV infarct which requires optimal preload to maintain cardiac output.
