This patient is a 38 year old female who presents to the ED right wrist pain after falling. The following x rays are shown.

1. Describe what you see?
2. What is the mechanism of injury?
- The mechanism of injury includes excessive hyperextension, ulnar deviation, and intercarpal supination.
3. What would you find on physical exam?
- Dorsal wrist swelling and decreased wrist motion especially flexion
- A palpable fullness may be noted on the dorsal side of the wrist
- Compression of the median nerve can occur in the carpal canal by the lunate, and the patient may display signs of a median nerve injury.
4. What would be your management for this patient?
- Immobilized with the wrist in neutral position in a volar splint
- Immediate Hand consult for reduction and definitive care.
- Pain management
